Deep plane facelift: working beneath the SMAS
A deep plane facelift dissects beneath the SMAS, the fibromuscular layer that supports the face, and releases the retaining ligaments. Skin, fat and SMAS move as a single unit and are repositioned along a vertical vector, upwards and slightly back, without the skin carrying the tension.
That difference in plane explains why the midface and the nasolabial fold respond differently than they do to a skin or SMAS facelift. It also sets its limits: it does not improve skin quality or replace bone volume, and in a face with good cheek support whose problem is concentrated in the neck it adds little.
| Technique | What it treats | Indicative recovery |
|---|---|---|
| Mini jowl lift | Jawline and early laxity | 7 to 12 days |
| Deep plane facelift | Moderate or advanced laxity, midface, nasolabial folds | Stitches out between 10 and 14 days |
| Deep plane neck and facelift | Midface, lower face and neck | Between 12 and 14 days |
| Platysma correction | Neck bands and fat under the chin | Between 7 and 10 days |

Rhinoplasty
Rhinoplasty changes the shape of the nose and, when needed, how it breathes, by working on the bone and cartilage that support it. It can be primary, on a nose that has never been operated on, or secondary, on one that has.
Dr. Verbauvede has used the ultrasonic instrument (Piezotome) in rhinoplasty since 2011 for work on the bone, in the cases where it helps. What can be changed depends on the anatomy of each nose, and the assessment may conclude that the right advice is not to operate.
- 01 Rhinoplasty What it corrects, techniques, recovery and when not to have surgery.
- 02 Ultrasonic rhinoplasty Work on the nasal bone with piezoelectric instruments.
- 03 Revision rhinoplasty Correcting the result of a previous rhinoplasty.
- 04 Non-surgical rhinoplasty Refining the profile without surgery, and how far it can go.
- 05 Profileplasty Nose and chin assessed as a single profile.

Other facial procedures
- 01 Blepharoplasty Removes excess skin from the upper eyelid and treats lower eyelid bags, when the tired look comes from the eyelid itself.
- 02 Brow lift Repositions the brow and forehead when the brow drops and pushes the eyelid skin down.
- 03 Lip lift Shortens the distance between the base of the nose and the upper lip. It changes the proportion without adding volume.
- 04 Otoplasty Corrects the position, size or shape of the ears, with the scar in the fold behind the ear.
- 05 Facial rejuvenation A plan that combines procedures according to the layer of the face most affected. Doing all of them is rarely indicated.
- 06 Facial bioosteoplasty Non-surgical treatment with biphasic bioceramic on the bone support of the cheekbone, jaw angle and chin.

How the process works
-
Assessment
In person in Madrid, Villagarcía de Arosa or Pozuelo, or online for a first orientation. It leads to the plan, or to the advice not to operate. Before any procedure, the in-person visit is essential.
-
Plan and anaesthesia
The type of anaesthesia depends on the extent of the surgery: local, local with sedation or general, the last with one night in hospital.
-
Surgery
Surgical procedures take place in a hospital. The assessment tells you which one applies to your case.
-
Follow-up
The first check-ups are in person because stitches, dressings or splints are removed. Later follow-up can often be done remotely, with photographs.

Frequently asked questions
What is a deep plane facelift?
It is a facelift that dissects beneath the SMAS and releases the retaining ligaments of the face. Skin, fat and SMAS are repositioned together along a vertical vector, with less tension on the skin. It acts mainly on the midface and the nasolabial fold, and it does not improve skin quality.
Can the first consultation be online?
Yes. The first assessment can be remote, to guide the case and answer questions. Before any procedure the in-person visit is essential, because the examination is what distinguishes between the treatment options.
Can I have the consultation in English?
Yes. Dr. Verbauvede and his practice see patients in English and in Spanish, both in the in-person consultation and in the online assessment.
Where is the surgery performed?
Surgical procedures take place in a hospital. The assessment tells you which one applies to your case.
At what age is a facelift considered?
There is no right age. The examination decides: the degree of sagging, the quality of the skin and what bothers the patient. Some people have an indication in their late forties, and some faces at sixty do not.
When can the result of a rhinoplasty be seen?
In a primary rhinoplasty, the result is assessed at one year. The tip is the last area to lose its swelling, and in thick skin or revision surgery the final details appear up to 18 or 24 months.
Can the doctor advise against surgery?
Yes. The assessment may conclude that the expected improvement does not justify the risk, that it is not the right time or that the problem can be solved another way. Leaving the consultation with the advice not to operate is a useful answer too.
